Short description how to Request tissue samples

Short protocol for the request of tissue samples (a more detailed description can be found in the Erasmus MC regulatory document)

Short protocol for the request of a FFPE, Frozen tissue samples or fresh (a more detailed description can be found in the Erasmus MC regulatory document) .

Fill out the tissue request form questionnaire

  • Personal details
  • Experimental details
  • Which tissues and how much of those tissues are required
  • Compliance to rules and regulations

The requested tissues and data will be given upon a positive evaluation. Therefore 1) the questionnaire needs to be completed, 2) the application needs to comply with the rules and regulations involved in scientific research with patient material and 3) a positive consensus within Tissue Bank Committee, which consists of the following members:

  • Head Erasmus MC tissue bank
  • Specialized pathologist responsible for the requested tissue type
  • Research specialist(s) frequently involved in requests on the requested tissue type.

Send in the completed form to the  PARTS  or the Head of the Erasmus MC Tissue Bank.

After a positive evaluation the samples and data can be issued to the requestor. On that moment a  Material Transfer Agreement (frozen tissue)   has to be signed by both supplying and receiving parties. This agreement holds the duties conditions for both parties concerning the samples transfer.
