Before attending your first appointment at the hospital, you must first register at one of our registration desks. We will take a photograph of you as part of the registration process. Our registration desks are open from Monday to Friday, from 7 am to 5 pm.
Either follow the signs for ‘Inschrijven’ (‘Registration’) or ask one of our hosts or hostesses to show you the way. The registration desks are also marked on this map. They are shown as large white spaces with the word ‘Inschrijven’ written in them.
Once you have registered, you will be issued with a day ticket showing the time and place of your appointment(s). One of our hosts or hostesses will be pleased to show you the way.
You will need the following in order to register:
- A valid ID document;
- Proof of health insurance;
- your GP’s name and address;
- The name, address and fax number of your pharmacy.
Your email address
You will also be asked to give your email address, which we use for informing you about matters concerning your treatment.
Patient satisfaction survey
We also use your email address to ask you about your experience with our healthcare services. What particular aspects were you pleased with – and which aspects were you less pleased with? The views of our patients are an invaluable tool in improving our services. Would you be willing to complete the occasional patient satisfaction survey? If so, we will send you an email from time to time with a questionnaire. Please note that your answers are anonymous and that you are not under any obligation to complete these surveys.
If you would prefer not to take part in these surveys, you can make this clear when you register with us and we will respect your decision. If you are already registered as one of our patients and if we mistakenly send you an email enclosing a patient satisfaction survey, you can click on the link in the email to unsubscribe. We will then use your email address only in order to inform you about matters concerning your treatment. You can always resubscribe to the satisfaction surveys at a later date, should you wish to do so.
Letter of referral
Please remember to bring a letter of referral from your GP or specialist with you if we do not already have it. If you don’t, there is a risk that your health insurance company may not cover the cost of your treatment and that we will have to invoice you instead.
If you have already registered with us and come for a follow-up appointment, please go to one of the self-service registration kiosks to obtain your day ticket.
How to change your personal details
You can use My Erasmus MC to change your personal details (such as your telephone number, email address and the details of your GP and pharmacy).
You can also use the contact form at the bottom of this page to pass on any changes in your home address, telephone number, GP, pharmacy or health insurance. Remember to state your patient number. Please do not include any medical details (confidential or otherwise) when filling in the contact form. You can also go to the registration desk in the main hall to report a change in your personal details.
Please check your personal details once a year to make sure they are still up to date.
